Independent Medical Consultation (IMC)
A one-time, advisory consultation for clarity and direction. Ideal for second opinions, a review of recent results, or a decision you are weighing.
Duration & Fee:
- 60 minutes: $500
- 90 minutes: $700
Includes:
- Review of your concerns and goals
- Review of records and labs uploaded by the patient before the visit
- Medical interpretation and clear recommendations
- Clear next steps, including when to involve your PCP or specialists
Does NOT include:
- Physical examination
- Ordering new labs or imaging
- Prescribing medications
- Acting as a primary care physician
- Ongoing follow-up, refills, or monitoring
Patients should maintain an established primary care clinician for ongoing care, prescriptions, and urgent needs.
Diagnostic Assessment
For patients who want a comprehensive medical evaluation and a clear plan.
- Visit 1 (120 minutes): Full history, physical examination, review of prior records +
- laboratory testing ordered
- Electrocardiogram (if indicated)
- Inbody Body Composition Scan
- Visit 2 (120 minutes): Results reviewed in detail, diagnoses explained, risks put in order of what actually matters, and a treatment plan.
- Laboratory testing included: comprehensive metabolic panel, complete blood count, HbA1c, fasting insulin, full lipid panel including ApoB and Lp(a), thyroid function, hs-CRP, iron studies, vitamin D, vitamin B12, Folate, and urine albumin-to-creatinine ratio. Additional testing, where clinically indicated, is billed at our negotiated rates.
Fee: $2,450
Includes above described laboratory panel and diagnostics.
If you join the practice within 90 days, this assessment counts as your comprehensive evaluation for the year — and your first five months of membership dues are waived.
The Metabolic Health Program
Six months of physician-led care for weight and metabolic health.
Most weight-loss services will write a prescription and leave the rest to you. Getting the prescription is the easy part. What comes after is harder: whether the dose is right, what and how to eat when nothing appeals, how to keep your muscle while the weight comes off, and what happens if you stop. Those are the questions this program exists to answer.

- A 90-minute assessment with laboratory testing and body composition
- Medication management, including prescribing where appropriate
- Individual reviews at month three and month six
- Six monthly group sessions with Dr. Yeganeh — medication and side effects, eating well on a smaller appetite, protecting muscle, plateaus, sleep and stress, and planning what comes next
- Repeat laboratory testing and body composition at months three and six
- Messaging between visits, answered within one business day
On medication: we don’t sell it. If you need a prescription you’ll fill it at your own pharmacy at your own cost, and we have no financial stake in what we prescribe.
Program Fee: $895 to enroll, which covers your assessment and your first month. Then $345 a month for the remaining five months. Laboratory testing is included.
Groups begin in January, April, July and October.
After the six months
Some people finish the program and go back to their own physician with a plan. Others want to keep going. Continuing care is $245 a month and includes a review every three months, repeat labs and body composition twice a year, ongoing prescribing and adjustment, and messaging between visits. Month to month, cancel any time.
Primary Care Membership
For patients who want an ongoing primary care relationship with direct physician access and proactive management.
Includes:
- Unlimited in office or telehealth visits (excluding house calls) for new concerns and ongoing conditions
- Prescribing, medication monitoring, and coordination of care
- Direct access to your physician: messages answered the same or next business day.
- A comprehensive evaluation when you join, and an annual review every year after
- Annual laboratory panel and body composition
- Two house calls a year within ten miles (additional house calls can be coordinated for a fee)
- Additional laboratory testing at our negotiated rates
- No insurance billing, no copays, no surprise invoices
Fee: $495/month
Membership begins with a comprehensive assessment, so that ongoing care is built on a complete picture rather than a first impression.
Dr. Yeganeh takes a limited number of new patients each year. Fewer patients means more time for each one.
Insurance & Payment
Healthy Conditions does not bill insurance and is opted out of Medicare. You pay us directly, and what is listed is the full cost of your care for that particular service.
Laboratory testing is included in the Diagnostic Assessment, the Metabolic Health Program, and your annual review as a member. Any additional testing is billed at our negotiated rates plus a small processing fee (usually a fraction of what a hospital system charges for the same test.)
Two house calls per year (within ten miles) are included in the Primary Care Membership. Additional house calls or larger radius is available and billed separately on an individualized basis.
