Duration & Fee:
- 60 minutes: $450
- 90 minutes: $650
Includes:
- Review of your concerns and goals
- Review of records and labs uploaded by the patient before the visit
- Medical interpretation and clear recommendations
- Clear next steps, including when to involve your PCP or specialists
Does NOT include:
- Physical examination
- Ordering new labs or imaging
- Prescribing medications
- Acting as a primary care physician
- Ongoing follow-up, refills, or monitoring
Patients should maintain an established primary care clinician for ongoing care, prescriptions, and urgent needs.
Diagnostic Assessment
For patients who want a comprehensive medical evaluation and a clear treatment plan.
Includes:
- Visit 1 (120 minutes): Intake, focused physical exam, review of history, and ordering appropriate labs when indicated
- Visit 2 (120 minutes): Results review, diagnoses, risk prioritization, and a clear treatment plan
Fee: $1,850
Laboratory testing, when indicated, is billed directly by the lab and may be covered by insurance or available at discounted self-pay rates depending on testing.
Foundational Health Assessment Program
A structured, physician-led evaluation with diagnostics, treatment when appropriate, and implementation support over approximately 8 to 10 weeks.
Includes:
- Two 120-minute physician visits
- Two 30-minute follow-up visits
- Prescribing when clinically indicated
- Secure messaging during the program for clarification of recommendations made during scheduled visits
- Clear exit plan and care handoff guidance when needed
Program Fee: $2,750

Concierge Primary Care Membership

For select patients seeking an ongoing primary care relationship with direct physician access and proactive management.
Includes:
- Unlimited visits for new concerns and ongoing conditions
- Prescribing, medication monitoring, and coordination of care
- Direct access to your physician for timely guidance
- Comprehensive primary care without insurance billing
Fee: $495/month
Availability: Offered on a limited basis by invitation only,
after completing an initial service.
